Terms and
Conditions of the IDSafeChoice Program

You may access services under the IDSafeChoice
Program within twelve (12) months from the date shown on your credit
report. This is entitled the “Subscription Period.”
·
Persons who are
eligible for this benefit are called “Subscribers” and include a person
or persons who are eligible for benefits as defined by the program.
Businesses and business accounts are not eligible for benefits under this
program.

The benefits
under this program are non-refundable, non-cancelable and non-transferable.
Benefits not utilized during the Subscription Period will expire with no
cash value.

For purposes of
benefits under this program, identity theft is defined as fraud that
involves the use of a Subscriber’s name, address, social security number,
bank or credit/debit card account number, or other identifying information
without the knowledge of the Subscriber, which is used to commit fraud or
other crimes.

Eligibility for
recovery services under the IDSafeChoice Program is based on identity theft
events that occur, are discovered, and reported to the named provider during
the Subscription Period. Persons who have identity theft events that
occurred prior to the beginning of the Subscription Period are not eligible
for benefits under this program, but may purchase benefits from the
provider.

Benefits are
available only to residents of the United States. In the event of fraud that
occurs outside of the United States, identity recovery is only performed
with agencies and institutions in the United States or territories where
U.S. law applies.

The provider
will not provide credit counseling or repair to credit that legitimately
belongs to a Subscriber.

The provider
may refuse or terminate services under this agreement if it is deemed that a
Subscriber is committing fraud or other illegal acts, making untrue
statements, or failing to perform his/her portion of the recovery plan. The
provider will not refuse or terminate services based solely on the
complexity of the case.
·
Identity Theft Recovery Services are provided by Merchants Information
Solutions, Inc. or other selected provider, at the discretion of the
National Credit Reporting Association, Inc.
